Moms In Motion (Moms)/At Home Your Way (AHYW) is a Consumer-DirectedServices where the person using them is allowed to select, hire, fire, and train their particular caregiver(s). More Service Facilitation provider for Virginia Medicaid waivers: CCC PlusA Virginia Medicaid waiver program that provides services to elders and people with physical disabilities in the community (as opposed to in a nursing home) to prevent the premature institutionalization of these groups. Services available via this waiver include assistance with the Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), such as bathing, grooming, dressing, light housecleaning, preparation of meals, and monitoring of medications. More & DD (CL & FIS). We serve Virginians of all ages.
Role of the Service Facilitator (SF):
A Consumer-Directed (CD) Services Facilitator (SF) is the DMAS-enrolled provider who is responsible for supporting the person and family/caregiver by ensuring the development and monitoring of the consumer-directed services plan of care, providing Employer of Record (EOR) management training, and completing ongoing review activities as required by DMAS for consumer-directed personal care and respite services.
